Eighteen-year-old Maddie says she doesn't have an eating disorder even though her mother, Melissa, stepfather, Greg, and grandmother, Linda, claim Maddie is slowly starving herself to death. Standing tall at 5'10, Maddie says if it wasn't for her family, she would weigh less than 100 pounds. Her mother says Maddie has been in and out of the hospital every other month for dehydration, can't get up a flight of steps without being out of breath, passes out in the shower, sleeps all day, and even quit school. But Maddie says everyone needs to calm down because she's in control. Does Dr. Phil think Maddie is in control or in need of help?

Penny says when she lost her close friend, Glena, she offered comfort to Glena's grief-stricken husband, Glen, but was shocked when he leaned over and tried to kiss her on the day of the memorial service. Within days, Glen says he revealed his true feelings for Penny, but she says she felt like she was betraying her dead friend. Nonetheless, just three months after the funeral, Penny and Glen were married. But Penny also fell ill and claims while she was in the hospital, Glen told her best friend, Paula, that she was dying. Not only that, Penny claims he asked Paula to move in, replace her as a mother to their now 4-year-old daughter, and marry him, which Glen denies. Although Penny survived — she says she doubts her marriage can. Emotions run high when Paula confronts Glen, resulting in him having a panic attack, causing emergency services to rush to the studio.

Twenty-year-old Laurie Depies vanished without a trace 26 years ago from her boyfriend, Marc's, apartment parking lot at around 10 p.m. The only clue she left behind was a cup on the roof of her 1984 Volkswagen. Her mother, Penny, is not a fan of her daughter's inmate marriage, and she says she's worried for her daughter's safety.

Althea says she feels like a prisoner trapped in a rage-filled relationship with her partner, Kevin, and has resorted to secretly taping his anger to have proof; Kevin says his rage is a result of brain trauma, and receives the results of an MRI.

Mark says he chooses to live in his beat-up car, held together with bungee cords and glue, because it gives him the freedom to travel, which broadens his horizons; his family says he needs to get his life together and stop acting like an adolescent.

Dr. Phil sits down with Kerri Rawson, author of A Serial Killer's Daughter: My Story of Faith, Love, And Overcoming. Kerri's father, Dennis Rader, is also known as the notorious serial killer "BTK," which stands for "Bind, Torture, Kill." BTK's murder spree terrorized Wichita, Kansas from 1974 to 1991, when he strangled 10 people while living a double life as a hands-on father, provider and churchgoer. Rader was eventually caught and arrested in 2005 and is currently serving 10 life sentences for killing eight adults and two children. Kerri talks about how her life was immediately shaken the day the FBI informed her who her father really was. Then, Dr. Phil speaks with Sherry Ballard whose daughter, Crystal Rogers, has been missing since 2015. During their search, Sherry's husband and Crystal's father, Tommy Ballard, was shot and killed. While both crimes remain cold cases, Sherry says she believes they are related. Former FBI Crime Profiler Candice DeLong looks at the clues and weighs in on what could have happened.

Kalan wrote into the show desperate for Dr. Phil's help because she says her fiancé, Giovanni, made a major shift in his life goals. Just two days after he asked Kalan to marry him, she says the Army veteran made a very serious decision to change his identity. Kalan says he has now taken on the persona of a superhero and will only answer to "Goose Wayne Batman." Kalan says he refuses to take off his mask or eye makeup and although she's loved him since she was 14, this may be too much to handle. "Goose," as he refers to himself, says he just wants to help and protect people, and being "Goose Wayne" is a non-negotiable part of his life and also his full-time job. The two are engaged to be married, but Kalan says she doesn't want to be walking down the aisle with a masked man.

Sixty-four-year-old Josie says she met the love of her life, Victor, on Facebook. Even though he's Nigerian, 30 years younger, and she's never physically met him, she insists it's true love. She's purchased a ticket to Nigeria and hopes to marry Victor, but her older sister, Carmen, her best friend, Angela, and her 19-year-old son, Anthony, say Josie is acting like a teenage fool. They believe Victor is conning Josie into marrying him for a green card. Dr. Phil took cameras to Nigeria to find Victor to see if he's telling Josie the truth. Josie's family says they want Dr. Phil to convince Josie that this relationship will never work. Is Victor who he says he is, and will Josie leave for Nigeria after hearing what Dr. Phil has to say?

Melissa says her 14-year-old daughter, Destoni, is aggressive, violent, and a bully. At home, she has chased her sister and babysitter with a butcher knife. At school, Destoni yells accusations at teachers in the hallways, throws school supplies, and gets kicked out of class almost every day. But Destoni crossed a line when she threatened to shoot her teacher and other students, causing her to be suspended from two different schools. Melissa and her mother, Michelle, fear Destoni will end up killed or homeless due to her behavior. One thing is for sure, Destoni is just not functioning well in life.

Dr. Phil continues his conversation with Melissa and her mother, Michelle, who say time is running out for 14-year-old Destoni. Melissa says her daughter's outrageous behavior will land her in jail, or even worse, get her killed. Melissa says the house is not safe when Destoni is there and wants to send her to foster care. Dustin, Chance and Nova claim their mother, Sonya, is a liar and manipulator who neglected them their entire lives. According to the children, they grew up in deplorable conditions with no food or hot water in their house. Dustin claims his mother would spend money on fast food for herself and her boyfriend, while his siblings were starving – and he even had to sneak food into the house to feed them. When Nova was 13 years old, she claims Sonya packed up and disappeared, leaving her and her then 14-year-old brother, Chance, homeless without a place to go. But Sonya says she does not remember starving her children or locking them out of the house. Will Sonya be able to earn a place in her children's lives again? Michelle and her daughter Brianna say her oldest daughter Tia – who recently changed her name to Rose – went from being a mom with a great job as a physician assistant to being unemployed, living in her truck and speaking with a British accent. Rose says she suffers from a rare condition called "Foreign Accent Syndrome," which she developed a few months ago after suffering a stroke. Rose says she "speaks like Mary Poppins with a bit of Nicki Minaj." But Michelle and Brianna say they are alarmed after witnessing some of Rose's behavior on social media that reveals more than just a British accent, such as singing to birds and children and wearing costumes from the 1800s. Michelle and Brianna say they haven't seen Rose in person in five years, but Rose says that has been her choice because her mother put so much pressure on her to be something she was not and now, she is living her most authentic life. Find out what happens when this family meets for the first time face-to-face in five years to try to figure out what is going on with Rose.

Terrie says in 2017, her daughter, Kelly, accused her of molesting her 5-year-old daughter, Terrie's granddaughter. Terrie insists this is completely untrue and insists she has never molested anybody. Terrie says Kelly is tearing apart the family with her horrific accusations. Terrie says just before Kelly's accusations began, she attended a week-long desert retreat that she believes completely changed her daughter's personality. Terrie says Kelly tells people she saw aliens on the retreat who may have abducted, raped and impregnated her. Kelly says she is 100% convinced her mom molested her daughter. She claims her mom is a serial child molester who has also molested her and her sister, Katelyn, which Katelyn and Terrie say is completely untrue. Kelly says she is tired of her mother using the desert retreat as a means to tell everyone in the family that's she's "crazy." She's hopeful that with Dr. Phil's help, she can finally expose her mother, whom she hasn't spoken to in over a year. Did Terrie molest her granddaughter? Is Kelly just confused?
